COST OF SERVICES

All new members must pay $125 (one time fee) at time of initial visit in addition to their regular monthly service fee which is based on age. 

• Full-Time College Students Primary Care (Less than 25 years of age):  $45 per month 
• Children 0-18 years of age: $35 per month 
• Adults 19-50 years of age: $50 per month 
• Adults 51-64 years of age: $75 per month
• Adults 65 years and older: $95 per month

YOUR MONTHLY FEE ALLOWS 3 VISITS PER MONTH! (Any Combination of in Person or Telemedicine) 

At this time Compassionate Care Primary Services, LLC only accepts Apple/Google Pay, CashApp, Debit/Credit Cards, and Florida Traditional Medicare and Medicaid as a form of payment.

Provider DOES NOT keep cash on hand. 

We DO NOT accept checks and insurance at this time, but you are still welcomed to join our group.
