COST OF SERVICES
All new members must pay $150 (one-time fee) at time of initial visit in addition to their regular monthly service fee which is based on age.
- Full-Time College Students Primary Care (Less than 25 years of age): $95.00 per month
- Children 10-18 years of age: $85 per month
- Adults 19-50 years of age: $100 per month
- Adults 51-64 years of age: $125 per month
- Adults 65 years and older: $145 per month
- Telemedicine Visit: $75.00 per visit
- Medication Evaluation: $45.00 per encounter
YOUR MONTHLY FEE ALLOWS 2 VISITS PER MONTH, IF NEEDED! (Any Combination of in Person or Telemedicine)
At this time Compassionate Care Primary Services, LLC only accepts Apple/Google Pay, CashApp, Debit/Credit Cards as a form of payment.
Provider DOES NOT keep cash on hand.
We DO NOT accept checks and insurance at this time, but you are still welcomed to join our group.

We are dedicated to providing high-quality medical services to all our patients. The goal is to ensure everyone receives the best possible care. Service fees listed above are for those living within a 20-mile radius. If greater than 20 miles, there will be an additional $100 fee added to your monthly membership.
